Likely, they started off OK, and the load didn’t get bigger, but rather they kept having to extend the boom, eventually causing the tip. Or, they couldn’t get the outriggers all teh way out on one side, and picked from the other, then swung around to the short side. It DOES happen much more often then one would think, and ALMOST happens MICH more that that.
Either way, it’s always FAIL. .

They did not over load the crane. The out riggers on the R/H/S were not all the way out as you can see from the photo.
